You are assigned to visit the home of a 75-year-old Samoan female… You are assigned to visit the home of a 75-year-old Samoan female after the personal care attendant (PCA) called to report a fall that occurred through the night secondary to the client attempting to use the commode without assistance. The client lives with her daughter and her 3 children ages 7, 12, and 14 years. The daughter currently works as the VP in a medical device company. The client’s son in law travels a lot for his work. The family lives in a high-rise apartment in downtown Minneapolis. Her medical history includes hypertension, osteoporosis, osteoarthritis, mild cognitive impairment, and type 2 diabetes. The client can ambulate with a walker. The PCA called after arriving at the client’s home to report the client fell during the night and sustained “minor injuries and scrape on her left shin”. The PCA stated the daughter reported the client fell at 0500, and states her mom was unsteady and slightly confused for the past two days. The client stated she was needed to use the commode last night and denies she lost consciousness. Her daughter states she helped her mom back to bed after she heard noise coming from her mom’s room. The PCA reports a 2 cm abrasion on her leg. During the call you instruct the PCA to monitor pain and mobility and let them know you will be coming to see the client this morning to evaluate her condition. Upon arriving at 10:30 you note that the client has a 2 cm abrasion on her right anterior leg, she moves all extremities without difficulty. BP 132/84, HR 96 and regular, RR 20, T 98.8 F (37.1 C) tympanic. PERRLA, blood glucose level is 112, you apply a clean dressing to the wound and provide wound care instructions to the family. The client tells you that she tried to get up to use the commode and got dizzy and fell, hitting her leg on the side of the commode. Breath sounds are clear, heart sound normal S1 and S2, last bowel movement yesterday, voiding without difficulty. Client is unsteady when walking with the walker and able to bear weight. States her pain level is a 2/10 on numeric scale and is her “usual arthritis pain”. A provider SBAR was completed, and orders were received for Tylenol 650 mg by mouth every 6 hours as needed for pain, urinalysis, and urine culture to be sent to the lab, CBC and Complete metabolic panel, PT evaluation and treatment plan.
