He reported no clear trigger for his respiratory symptoms, although he had noted some symptoms of a mild upper respiratory tract infection. With this episode, the child had been using a short-acting bronchodilator more frequently than he had in the past, without the expected resolution of symptoms. On the day of consultation, the child awoke feeling ‘suffocated’ and his mother noted circumoral cyanosis. In the emergency department, his oxygen saturation was 85%, respiratory rate 45 breaths/min, pulse rate 105 beats/min and a body temperature of 101.84 °F. He had significantly increased work of breathing and poor air entry bilaterally to both lung bases, with wheezing in the upper lung zones, with intercostal retraction and nasal flaring. The child was agitated and restless. The child’s weight was 35 lbs. He was treated with salbutamol/ipratropium and received intravenous steroids and magnesium sulfate. His chest x-ray showed hyperinflation and no focal findings. His medical history revealed that he was followed by a pulmonologist for his asthma, had good medication adherence and had not experienced a significant exacerbation for six months. He also had a history of wheezing, dyspnea and pruritis with exposure to peanuts, and chickpeas; he had been prescribed epinephrine for this. In the past, his wheezing episodes had been seasonal and related to exposure to grass and pollen. Further history revealed the probable cause of his consultation. ASSESSMENT:Subjective Data:He had noted some symptoms of a mild upper respiratory tract infection.The child awoke feeling ‘suffocated’.Objective Data:RR: 45 bpmPR: 105 bpmTemperature: 101.84 °F SpO2: 85%Weight: 35 lbs (severely underweight)Circumoral cyanosis was noted.Significantly increased work of breathing and poor air entry bilaterally to both lung bases, with wheezing in the upper lung zones.With intercostal retraction and nasal flaring.Chest x-ray showed hyperinflation.DIAGNOSIS Ineffective Breathing Pattern related to swelling and spasm of the bronchial tubes PLANNINGShort-term:After 4 hours of nursing intervention, the patient will:- Have improved breathing pattern as evidenced by decreased respiratory effort, normal pulse rate, increased SpO2 (at least 90%)- Be able to verbalize the understanding of appropriate measures to improve breathing patterns.Long-term:After 2 days of nursing intervention, the patient will:- Maintain optimal pulmonary ventilation, as evidenced by relaxed breathing, normal respiratory rate, and SpO2 level of 95% or greater.- Be free of respiratory distress as evidenced by absence of cyanosis, dyspnea, and nasal flaring.- Demonstrate behaviors that help in maintaining optimal respiratory status.EVALUATIONGoals are met.After 4 hours of nursing interventions, the patient:- Had improved breathing pattern as evidenced by decreased respiratory effort, normal pulse rate, increased SpO2 (at least 90%)- Was able to verbalize the understanding of appropriate measures to improve breathing pattern.
